Follow the link below or find the online scheduling tool on our website or Facebook page. Choose the type of appointment you need, preferred provider, and location  and see the list of available times. Once you’ve selected an appointment, you’ll get an email letting you know your appointment request has been submitted, and then another email when the appointment has been confirmed.
